Eligibility

These services benefit individuals who require assistance with activities of daily living (ADLs), including:

• Eating

• Dressing

• Bathing

• Toileting

• Mobility

To qualify for PCS, an individual must have a medical condition, disability or cognitive impairment, and demonstrates unmet needs for:

• Three of the five ADLs with limited hands-on assistance

• Two ADLs, one of which requires extensive assistance

• Two ADLs, one of which requires assistance at the full dependence level

PCS program eligibility is determined by an independent assessment conducted by NC Medicaid or its designee, and is provided according to an individualized service plan.

Respite Care Services offer temporary, short-term support for unpaid primary caregivers of individuals with intellectual disabilities, mental health needs, or physical impairments. These services provide crucial caregiver relief by temporarily taking over daily care duties.

Primarily provides emotional support, socialization, and supervision. Duties include light housekeeping, meal preparation, medication reminders, and transportation. Companions do not provide direct hands-on physical help with bathing, dressing, or toileting.

The Community Alternatives Program (CAP), is a set of Medicaid waiver programs that help medically fragile individuals live at home instead of in nursing homes or hospitals. The two main waivers are CAP/DA for adults (18+) and CAP/C for children (0–20)

The Two CAP Programs

• CAP/DA (Disabled Adults): For physically disabled adults and seniors (over 65) who require a nursing facility level of care. It covers services like personal care aides, adult day health, and home accessibility modifications. Your household income is not counted—only the applicant's income is considered, allowing you to qualify even if you exceed regular Medicaid limits.

• CAP/C (Children): For medically fragile children up to age 20 who meet an institutional level of care. This program provides attendant nurse care, in-home aides, and specialized medical equipment. Eligibility is based on the child's income only.

How to Apply and Get Started

1. Request a Referral: You or a caregiver can make a referral directly by calling NC LIFTSS or by contacting your local Department of Social Services (DSS).

2. Needs Assessment: A case manager will conduct an evaluation to determine your functional and medical needs.

3. Once approved: Let your caseworker know that you want Johnson Family Home Care to provide in-home personal care services. Or, contact our office and a team member will assist.
